• Address Change

     

    Your address change must be submitted directly to the Student Services Department via one of the following methods.  (Please do not submit to your school as that will delay processing)

        • fax 816-736-5305
        • scheduled appointment - please call 816-736-5310 to schedule an appointment

    The following documents are required to update your students' address:

    Address Form

        • Must be completed by a legal parent/guardian
        • Please include all students attending Liberty Public Schools.

    Photo ID of legal parent/guardian

    Acceptable proof of residency All documents must be dated within the last 60 days and must be in the legal parent/ guardian's name.

    Two different utilities - Water, Gas or Electric only

          • Including the portion of your bill with the service address, date and name

    Or

    Lease Agreement

          • Including the pages with your name, lease term, property address and signature page with the landlord

    Or

    Non-Contingent home sales contract

    Or

    Settlement statement from home purchase
