Transition Services
Post-secondary transition planning is a critical component in supporting students as they plan for post-secondary goals. The planning process assists students in recognizing and preparing for post-secondary goals such as employment, post-secondary training, and independent living. Transition assessment and planning offer the student the opportunity to identify and share their strengths, goals, and interests and outline a plan to achieve their goals for the future. These goals are then supported by the school, parent, and outside agencies (when appropriate). Transition planning in Liberty Public Schools begins in the 8th grade, and plans are reviewed and updated annually.
