Transfer Request

Because this is a web-based form it must be completed and submitted electronically

• If you need assistance with completing and submitting this form please contact your students School or Student Services.
• Important - Depending on your transfer request reason you may be required to submit additional documentation with the Transfer Request. 
  • Please ensure you have these documents completed, prior to the submission of the Transfer Request, so that you  are able attach in the "file upload" section of the Transfer Request form.  
    • Transfer Request - based on "Childcare" you will also need to submit the "Childcare Notarized Letter" (located under Forms)
    • Transfer Request - based on "Change of Address"  - you will also need to submit the "Address and Demographic Change Form" with supporting documents (located under Forms)
• You may sign electronically
• Upon submission of the form with applicable attachment(s):
  • You will receive a confirmation of submission, plus, an email with your submission details
  • Student Services will automatically receive your Transfer Request
