Important - Depending on your transfer request reason you may be required to submit additional documentation with the Transfer Request.
- Please ensure you have these documents completed, prior to the submission of the Transfer Request, so that you are able attach in the "file upload" section of the Transfer Request form.
  - Transfer Request - based on "Change of Address" you will also need to submit the "Address & Demographic Change" form, Photo ID and residency proofs.
  - Transfer Request - based on "Childcare" you will also need to submit the "Childcare Notarized Letter" (located under Forms)

Upon submission of the form with applicable attachment(s):
- You will receive a confirmation of submission, plus, an email with your submission details
- Student Services will automatically receive your Transfer Request

If you have recently submitted a "New Student Enrollment", please contact your school of residence and let them know you are planning on submitting a transfer request.
You may submit a transfer request to the Student Services department, but, we will not review and provide a determination until the new student enrollment is finalized.

If approved the transfer may be revoked if these conditions are not met or occur:

Student must maintain regular attendance of at least 90%
Student must maintain satisfactory conduct
Student must maintain satisfactory schoolwork
Students classroom becomes overcrowded

If approved, it is for this school year only and any further request will be considered at the time the request is made.

We certify the information given above is accurate and any misrepresentation of facts could void an approved transfer.

Transportation: Parents/guardians are responsible for student transportation to and from school with the exception of childcare transfers.

All MSHAA Rules/Regulations apply.
